It could have been as simple as a kind word with an understanding smile or as significant as a life-saving procedure that made your visit to Canton-Potsdam Hospital extraordinary.  Either way we would love to hear about your experience and what made a difference to you.  Sharing appreciation can be very rewarding for everyone involved.

At Canton-Potsdam Hospital, patient care is a priority for each member of our healthcare team.  You and your family deserve the best health services and we strive to exceed your expectations each day.

Many patients have asked us how they can give back to Canton-Potsdam Hospital in honor of the "exceptional" care received.  The Canton-Potsdam Hospital Foundation has established the Grateful Patient Program to provide a special opportunity for patients and family members to express their thanks.

With the generosity of people like you, the Grateful Patient Program supports the purchase of medical equipment, upgrading our technology, and improving our facilities.

Whether you choose to salute a special doctor or nurse, an entire department, or a clinical program that impacted your health or that of a loved one, your donation is a meaningful opportunity to say "thank you", and to give back to those who may have given so much to you.

If you'd like to participate in Canton-Potsdam Hospital's Grateful Patient Program, you may click on the "donatenow" button on the right hand side of the page.  You may also print out the sheet and complete the form and mail it to:  Canton-Potsdam Hospital Foundation, 50 Leroy Street, Potsdam, NY 13676.

Even if you unable to give at this time, you may use the space provided on the form to send a thank you note to an outstanding doctor or member of the CPH team or email us at foundation@cphospital.org.  We will forward your note of appreciation to them.  When recognizing individuals, please specify their full name and department.
