When you are a patient of Canton-Potsdam Hospital, your care is our number one priority. Our Patient-Centered Medical Home model was designed to deliver healthcare services that specifically meet the needs of our communities. Whether you need a routine check-up or want to make an appointment with a specialist, you now have access to comprehensive primary care that offers affordable, efficient and high-quality healthcare where you need it, when you need it.

We are constantly working to provide quality healthcare to meet our patients' needs. Potsdam Primary Care Center is nationally certified in this Patient-centered Medical Home care model. We are working toward national certification for all other primary care health centers. This model is based on the following principles:


Access a wide range of medical services at our Primary Care Centers so you don't have to wait in the Emergency Room for health-related issues that are not emergencies and so you can establish a baseline for future health needs. Our Health Centers offer Urgent Care facilities, laboratories, radiology services, and easy coordination for follow-up appointments, even for patients without a regular Primary Care Provider. All patients have 24-hour access to their available medical records online through our outpatient and inpatient portals. To sign up, call or ask a staff member at your Primary Care Center for assistance. We are here for you, when you need us.

Comprehensive Care Team

Comprehensive patient care requires a team of dedicated healthcare professionals, which is why our team of care providers (both physical and virtual) meets daily to discuss upcoming appointments. This team is accountable for each patient's physical and mental healthcare needs. Our entire team also meets monthly to discuss factors that could impact patient care across the practice. We are trained to work as a team to provide comprehensive care.

Patient-Centered Care

Every patient is different, and each one deserves individualized care to meet all of their healthcare needs. Our relationship-based Patient-Centered Medical Homes take into account everything from your prior health history to cultural values and religious beliefs to bring you the most comprehensive healthcare available. We want to encourage our patients to take an active role in their healthcare, to ask questions, and have a better understanding of their overall health. We want to get to know you so we can provide optimum healthcare tailored to you.

Coordination Across Environments

Our Primary Health Care Centers are organized, share documentation where appropriate, and frequently communicate across all relevant providers, including nursing homes, home-care agencies, Public Health, and other community health agencies and partners. Our team of Transitional Care Managers and Referral Coordinators work to ensure each patient's appropriate information reaches appropriate care providers. This allows our patients to have a home base for all healthcare needs and reduces inefficiencies such as duplicate testing and procedures. We want to be sensitive to your schedule.

Commitment to Safety and Quality

Our model requires a commitment to high-quality care for all of our patients. To ensure this, a team of informed volunteer patients meets monthly to discuss the patient experience, quality, and care coordination at our facilities. This allows us to consistently measure and adjust our healthcare services and improve the patient experience. We put ourselves in your shoes.
