Inpatient/ER Patient Portal Access:

Your first step is to ask a patient representative at the registration desk of the main hospital at 50 Leroy St. to sign up. Then, follow the link below for access to available inpatient or ER visit records: you will be prompted to enter the unique log-in ID you received during your stay:

Outpatient Practice Patient Portal Access:

Canton-Potsdam Hospital has two Patient Portals to service their patients dependent upon their selected provider. These portals will provide access to patient information such as history, lab results and important Doctor/Patient communications.

Patient Portals are for Inpatient/ER Patient Portal Access and for Outpatient Practice Patient Portal Access. To access your personal Patient Portal information you must first create an account through you current doctor's office. Once your account is created, you may access your account by selecting your specific doctor from the list below. You will then be taken to the portal page where you can log in to review your personal information.
