Rehabilitation

We are a 17 bed unit with a variable length of stay program that provides a medically monitored 24 hour structured setting.  Seven professional staff (including CASAC, RN, CSW, CTRS) act as primary counselors and are involved with group and individual counseling as well as educational sessions. 

 

Our newly created pet therapy is part of the program and those with allergies, extreme asthma, or phobias should probably select another program.  For everyone's health, effective, July 1, 2008, CPH has become a tobacco-free facility.  Smoking is prohibited in or on any Hospital property, including parking areas.  

 

Our program includes the following:

  • Group and individual counseling
  • Family counseling and education
  • Trauma group therapy
  • Occupational and recreational therapy
  • Aftercare planning
  • Self Help meetings
  • Health education
  • Smoking cessation
  • Gambling assessment and counseling
  • Pet therapy

Pet Therapy:

The purpose of this program is to enhance residents’ quality of life through visual, physical, tactile, emotional and social stimulation and provide a “home-like” atmosphere. Therapy pets include residing cats and visiting dogs.  


Intake/Admissions: 

Please call ahead to determine availability and to complete a preadmission screening.  If the patient is being referred from a detoxification unit or other medical setting, the following will be requested:

 

  • Physician’s History and Physical Report
  • Psychosocial History
  • Labs, X-ray, and EKG Reports
  • Medication Records
  • Discharge Summary
  • Disclosure

Referrals are accepted from: healthcare agencies, legal agencies, DOSS, self, family/friends, outpatient services and mental health agencies.

 

Fees for services: Medicaid and most private insurances are accepted.   ** Insurance needs to be open/active prior to admission. **

 

Hours of Admission:

  • Referrals can be accepted Monday through Friday from 8:00 a.m. to 4:00 p.m.
  • Admissions are scheduled Monday through Friday ideally between 8:00 a.m. and noon.

 Criteria for Admission:

  • A diagnosis of chemical dependence and an inability to abstain without care in a structured setting; OR
  • A lack of appropriate outpatient services to meet the individual’s needs; OR
  • A lack of an appropriate living environment from which to receive services.

Readmission:

Generally, readmissions are not accepted within a year of discharge.

 

Unable to Admit:

We are unable to admit patients with the following:

  • Any medical or psychiatric condition requiring acute care;
  • A mentally ill chemical abusing (MICA) patient who is not well-stabilized on medications.

Early Discharge:

Administrative or early discharge will take effect for any of the following reasons:

  • Fraternization;
  • Acute medical/psychiatric illness requiring a higher level of care;
  • Inability to respond to the treatment services provided;
  • Behavior that is disruptive to the group’s progress;
  • Non compliance with program rules;

Continuing Care:

Discharge planning for patients in rehab focuses on referrals for outpatient treatment or, in some cases, community residential living.

 

Family Information:

Visiting Hours are on Sundays from 1-5:00 p.m. 

Telephone calls are allowed from 6:00 a.m. to 8:00 p.m. only during free time.  Calls are limited to 10 minutes and are not intended to be daily.  For the first three days of their stay, patients will not be allowed to use the phone. All mail may be sent to: 50 Leroy Street Potsdam, NY 13676.

 

Upon admission, you will receive a patient handbook that will explain accommodations, personal items and policies in more detail.
